Complete your Donor History Questionnaire on the date of your donation
Quick Pass
Quick Pass allows prospective donors to enter their donor history questionnaire responses on the day of their donation before arriving at the donor center or mobile blood drive.
Please bring your questionnaire completion confirmation barcode to your scheduled appointment. When you arrive at the blood donation site, please show the confirmation barcode on your mobile device or bring a printout so that our Team Members may scan it to register you.
If you do not bring your confirmation barcode, you must complete the questionnaire again.

- Bring printed or mobile confirmation to your scheduled appointment

Valid only on the date identified on completion confirmation. Successful completion of Quick Pass does not guarantee eligibility to donate; verification of eligibility will be determined on location at the time of donation.
